Lab Testing Benefits
Lab Card
Offered through Quest Diagnostics®, the Lab Card® program allows
employees and their eligible dependents the option of having all
covered outpatient laboratory testing done at no
cost. Nippon Life Benefits® pays the full amount of
covered and approved laboratory services - which means employees
incur no deductibles, copayments, or coinsurance costs.
Lab Card Select
The Lab Card® Select program offers discounted outpatient
laboratory testing to customers with High Deductible Health Plans
(HDHP).
General Information
In order to use these optional programs, the employee's
physician must order the testing and all services must be covered
and approved by the employee's health care plan. Quest
Diagnostics utilizes fully accredited, certified, and licensed
facilities that perform most diagnostic outpatient laboratory tests
ordered by a treating physician and covered by a Nippon Life
Benefits plan.
Included are:
- Blood, cholesterol, and urine tests
- Pap smears
- Biopsies
- Cultures
- Other cytological and pathological tests
