flexibility to design programs to fit a specific need. Our
dental plans cover preventive, basic, major, and orthodontia (if
elected). Cost sharing and benefit limits provide an incentive to
use the plan wisely.
Our high-quality plans are convenient and affordable, and can
include full reimbursement for preventive care with no deductibles.
We offer a variety of packaged options, as well as the flexibility
to design customized solutions for larger employers.*
Employers can choose a voluntary or contributory plan.

Rollover Benefit**
In addition, our dental plan offers a valuable feature that
allows employees to carryover part of their unused annual maximum.
By rolling over unused dollars, employees can increase their
maximum benefit and lessen future out-of-pocket expenses.
How does the Rollover Benefit work? As long as the total
benefits paid for an employee do not exceed the Threshold Amount
(1/2 of calendar year maximum) in any year, he or she qualifies to
"rollover" benefits up to a maximum limit. Rollover over
options include 25%, 30% and 35%.
The additional benefit that can be accumulated under the
Rollover Benefit is the lesser of the annual maximum or $2,500.
Under most programs, employees would be able to double their dental
maximum over time.
The Rollover Benefit applies to Preventive, Basic, and Major
calendar year maximum.
